Meals On Wheels (Sudbury) does offer a subsidy program for our meal programs to those who qualify. If you are interested in receiving our meals but feel you might not be able to afford them, please contact our office at (705) 525-4554 for more information.

How to Apply
STEP 1
You, your doctor, your discharge nurse, your LHIN manager, your family, or your caretaker can contact Meals On Wheels (Sudbury). Every door leads to our services. You can reach us in person, contact us by phone, e-mail or fax, or apply online here. See Contact Us for full contact information. Once we receive a referral or your application form you will be contacted. We will give you (the client) a call to confirm through a verbal agreement to begin receiving our services.
STEP 2
You (the client) will be placed on Meals On Wheels (Sudbury) services within 24-48 business hours or as per your schedule.
STEP 3
You will be contacted by a Meals On Wheels (Sudbury) staff member to schedule an in-home visit with the Coordinator of Client Services, who will provide you with further details about the Meals On Wheels (Sudbury) services and its various programs. A nutrition and health status assessment will take place during the in-home visit. The in-home assessment tool is used to determine what plan is best suited for you and if other community services are needed. Within the assessment there will be a questionnaire, a briefing of client/ staff (volunteer) policies, a service plan, payment options and delivery services.
